Live donor kidney transplantation is the best treatment option for most patients with late-stage chronic kidney disease; however, the rate of living kidney donation has declined in the United States. A consensus conference was held June 5-6, 2014 to identify best practices and knowledge gaps pertaining to live donor kidney transplantation and living kidney donation. Transplant professionals, patients, and other key stakeholders discussed processes for educating transplant candidates and potential living donors about living kidney donation; efficiencies in the living donor (...) evaluation process; disparities in living donation; and financial and systemic barriers to living donation. We summarize the consensus recommendations for best practices in these educational and clinical domains, future research priorities, and possible public policy initiatives to remove barriers to living kidney donation. Objectives: The mental health legislation of most developed countries includes either a dangerousness criterion or an obligatory dangerousness criterion (ODC). A dangerousness criterion holds that mentally ill people may be given treatment without consent if they are deemed to be a risk to themselves or others. An ODC holds that mentally ill people may be given treatment without consent only if they are deemed to be a risk to themselves or others. This paper argues that the dangerousness criterion is unnecessary, (...) unethical and, in the case of the ODC, potentially harmful to mentally ill people and to the rest of the community. Methods: We examine the history of the dangerousness criterion, and provide reasoned argument and empirical evidence in support of our position. Results: Dangerousness criteria are not required to balance the perceived loss of autonomy arising from mental health legislation. Dangerousness criteria unfairly discriminate against the mentally ill, as they represent an unreasonable barrier to treatment without consent, and they spread the burden of risk that any mentally ill person might become violent across large numbers of mentally ill people who will never become violent. Mental health legislation that includes an ODC is associated with a longer duration of untreated psychosis, and probably contributes to a poorer prognosis and an increase risk of suicide and violence in patients in their first episode of psychosis. Conclusions: Dangerousness criteria should be removed from mental health legislation and be replaced by criteria that focus on a patient’s capacity to refuse treatment. (shrink)

In the recent past, several papers have pointed to the possibility that tumour removal generates a permissive environment in which tumour growth is potentiated. This phenomenon has been coined “perioperative tumour growth” and whilst it represents a departure in terms of our attitude to the surgical process, this concept was first hinted at by Paget1Sir James Paget (1814–1899) was a surgeon and physiologist who is widely held (along with Rudolph Virchow) to be the father of the science of pathology. Despite this, the time interval immediately before and after cancer surgery (i.e. the perioperative period) remains an underutilised interval during which chemotherapeutic regimens are rarely implemented. Herein, we present a summarised review of the literature that supports the concept that tumour removal may potentiate the growth of residual neoplastic disease. We also outline current knowledge regarding underlying mechanisms and in this manner highlight potential therapeutic entry points. Finally, we emphasise the urgent need for trials of agents that could protect patients against the harmful host–tumour interactions that may occur during the perioperative period. Supporting children to successfully transition from primary to secondary school is of utmost importance for several reasons, including to prevent future emotional and behavioural problems. Level Up is a novel, UK-based intervention consisting of five online group sessions, straddling the summer holidays, and providing at-risk children and their parents/carers with skills to manage their behaviour, emotions, and relationships to support their transition to secondary school. A prior evaluation of Level Up reported a need to better describe the mechanisms of change. (...) This study therefore evaluated the experiences of children and their parents/carers regarding the facilitators and barriers to engagement and change, and the perceived impact. Fourteen children and 17 parents/carers were interviewed. Identified barriers and facilitators were: (1) Having a safe, supportive, and fun space, (2) Learning through connection, (3) A family approach, (4) Problematic group dynamics, and (5) Connecting through video calls. Perceived impact was described as: (1) Empowering children, (2) Supporting children socially, (3) Supporting parents and carers in their parenting role, and (4) Supporting a successful transition to secondary school. Another theme (5) describes some families’ experiences of limited impact. These findings can be used to better understand how to support children in their school transition. (shrink)

Spirituality is a universal human experience. Within the process of development, the role of spirituality as a developmental asset is understudied in general and especially within majority world contexts. In this article, we frame advances in spirituality research and practice with youth around three pillars: (a) theory, (b) measurement, and (c) research about and evaluations of positive youth development (PYD) programs in low- and middle-income countries. We place PYD programs as associated with dynamic, relational developmental systems (RDS)-based models of human (...) development, describe spirituality and the “Big Three” features of effective PYD programs as aligning to promote thriving, and discuss spirituality as a strength, as a key asset, in the lives of youth. We discuss advances in developmental methods and measures for describing, explaining, and promoting spirituality and PYD and offer a case study that responds to a United States Agency for International Development call for action and evaluations of spirituality within the context of a PYD program in the majority world. Considerations for future research about spirituality and youth development are discussed. (shrink)

An important challenge in bilingualism research is to understand the mechanisms underlying sentence processing in a second language and whether they are comparable to those underlying native processing. Here, we focus on verb-particle constructions (VPCs) that are among the most difficult elements to acquire in L2 English. The verb and the particle form a unit, which often has a non-compositional meaning (e.g., look up or chew out), making the combined structure semantically opaque. However, bilinguals with higher levels of English proficiency (...) can develop a good knowledge of the semantic properties of verb-particle constructions (Blais and Gonnerman, 2013). A second difficulty is that in a sentence context, the particle can be shifted after the direct object of the verb, (e.g., The professor looked it up). The processing is more challenging when the object is long (e.g. The professor looked the student’s last name up.). This shifted structure favors syntactic processing at the expense of VPC semantic processing. We sought to determine whether or not bilinguals’ reading time (RT) patterns would be similar to those observed for native monolinguals (Gonnerman and Hayes, 2005) when reading VPCs in sentential contexts. French-English bilinguals were tested for English language proficiency, working memory and explicit VPC semantic knowledge. During a self-paced reading task, participants read 78 sentences with verb-particle constructions that varied according to parameters that influence native speakers’ reading dynamics: verb-particle transparency, particle adjacency and length of the object noun phrase (NP; 2, 3, or 5 words). RTs in a critical region that included verbs, NPs and particles were measured. Results revealed that RTs were modulated by participants’ English proficiency, with higher proficiency associated with shorter RTs. Examining participants’ explicit semantic knowledge of VPCs and working memory, only readers with more native-like knowledge of VPCs and a high working memory presented RT patterns that were similar to those of monolinguals. Therefore, given the necessary lexical and computational resources, bilingual processing of novel structures at the syntax-semantics interface follows the principles influencing native processing. The findings are in keeping with theories that postulate similar representations and processing in L1 and L2 modulated by processing difficulty. (shrink)

Napolitano (The epistemology of fake news, Oxford University Press, 2021) argues that the Minimalist Account of conspiracy theories—i.e., which defines conspiracy theories as explanations, or theories, about conspiracies—should be rejected. Instead, she proposes to define conspiracy theories as a certain kind of belief—i.e., an evidentially self-insulated belief in a conspiracy. Napolitano argues that her account should be favored over the Minimalist Account based on two considerations: ordinary language intuitions and theoretical fruitfulness. I show how Napolitano’s account fails its own purposes (...) with respect to these two considerations and so should not be favored over the Minimalist Account. Furthermore, I propose that the Minimalist Account is the best conception of ‘conspiracy theory’ if we share Napolitano’s goal of advancing the understanding of conspiracy theories. (shrink)

Since the BBS article in which Premack and Woodruff (1978) asked “Does the chimpanzee have a theory of mind?,” it has been repeatedly claimed that there is observational and experimental evidence that apes have mental state concepts, such as “want” and “know.” Unlike research on the development of theory of mind in childhood, however, no substantial progress has been made through this work with nonhuman primates. A survey of empirical studies of imitation, self-recognition, social relationships, deception, role-taking, and perspective-taking suggests (...) that in every case where nonhuman primate behavior has been interpreted as a sign of theory of mind, it could instead have occurred by chance or as a product of nonmentalistic processes such as associative learning or inferences based on nonmental categories. Arguments to the effect that, in spite of this, the theory of mind hypothesis should be accepted because it is more parsimonious than alternatives or because it is supported by convergent evidence are not compelling. Such arguments are based on unsupportable assumptions about the role of parsimony in science and either ignore the requirement that convergent evidence proceed from independent assumptions, or fail to show that it supports the theory of mind hypothesis over nonmentalist alternatives. Progress in research on theory of mind requires experimental procedures that can distinguish the theory of mind hypothesis from nonmentalist alternatives. A procedure that may have this potential is proposed. It uses conditional discrimination training and transfer tests to determine whether chimpanzees have the concept “see.” Commentators are invited to identify flaws in the procedure and to suggest alternatives. The legitimacy of the dominant intensive meat production system with respect to the issue of animal welfare is increasingly being questioned by stakeholders across the meat supply chain. The current meat supply is highly undifferentiated, catering only for the extremes of morality concerns (i.e., conventional vs. organic meat products). However, a latent need for compromise products has been identified. That is, consumer differences exist regarding the trade-offs they make between different aspects associated with meat consumption. The heterogeneity in consumer demand (...) could function as a starting point for market segmentation, targeting and positioning regarding animal welfare concepts that are differentiated in terms of animal welfare and price levels. Despite this, stakeholders in the meat supply chain seem to be trapped in the dominant business model focused on low cost prices. This paper aims to identify conflicting interests that stakeholders in the meat supply chain experience in order to increase understanding of why heterogeneous consumer preferences are not met by a more differentiated supply of meat products produced at different levels of animal welfare standards. In addition, characteristics of the supply chain that contribute to the existence of high exit barriers and difficulty to shift to more animal-friendly production systems are identified. Following the analysis of conflicting interests among stakeholders and factors that contribute to difficulty to transform the existing dominant regime, different routes are discussed that may help and motivate stakeholders to overcome these barriers and stimulate the creation of new markets. Sleep onset is associated with marked changes in behavioral, physiological, and subjective phenomena. In daily life though subjective experience is the main criterion in terms of which we identify it. But very few studies have focused on these experiences. This study seeks to identify the subjective variables that reflect sleep onset. Twenty young subjects took an afternoon nap in the laboratory while polysomnographic recordings were made. They were awakened four times in order to assess subjective experiences that correlate with the (...) (1) appearance of slow eye movement, (2) initiation of stage 1 sleep, (3) initiation of stage 2 sleep, and (4) 5 min after the start of stage 2 sleep. A logistic regression identified control over and logic of thought as the two variables that predict the perception of having fallen asleep. For sleep perception, these two variables accurately classified 91.7% of the cases; for the waking state, 84.1%. (shrink)

The elegiac poem of eight lines discovered in the Ceramicus and published by by W. Peek is of considerable interest for the historian. Peek is surely right in maintaining that it was composed for the Athenians who fell under Tolmides at Coronea in 447 B.C., and his general exposition of the poem's meaning is convincing. The aim of this paper is to make some comments and supplements to his interpretation and then to consider some peculiarities in the thought and technique (...) of the poem. Vergil's plagiarism has been a theme for critics ever since Perellius Faustus made an anthology of his ‘furta’ and Quintus Octavius Avitus com-piled eight volumes of Оμоιóτησ, giving both the original passages and Vergil's adaptations of them . Much of this literature has survived in the commentary of Servius and in Book VI. of the Saturnalia of Macrobius. The study of his imitations and plagiarisms throws much light on Vergil's methods and aims of composition, and has frequently been attempted in (...) editions of his works. But one class of his plagiarisms differs considerably from the rest. Often in the Aenèid he incorporates phrases, either intact or slightly altered, from the Annales of Ennius. These differ from where he imitates Homer, because in them he does not have to translate from Greek into Latin and so remake the original into something new, and they differ, too, from his adaptations of some other earlier Latin poets, because Ennius was his only important forerunner who had written on the same heroic themes of war and the growth of Rome. The revival of interest in Greek metre cannot be without an influence on the text of Pindar. In some ways this influence may have been for the good, but in one respect a theory based on insufficient evidence seems in danger of corrupting the text. The theory is briefly that in his dactylo-epitrite poems Pindar occasionally equated a choriamb — — with an epitrite — — — or — — —. This theory has been stated explicitly by two leading Pindaric (...) scholars, Otto Schröder and U. von Wilamowitz-Moellendorf. In the Prolegomena to his Editio Maior, p. 13, Schröder briefly states his case for what he calls ‘metathesis metrica.’ Moved by the fact that in iambic and Ionic metres a short syllable may be substituted for a long, and vice versa, he is convinced that the same applies to the metres of Pindar. In his Works of Pindar, Vol. II, p. xxiii, Dr. L. R. Farnell discusses the admission of metrical licences into Pindar's text, and pronounces that ‘the “Responsion-law” should not be pressed with over-strained severity.’ In general he agrees with Wilamowitz and Schroeder and disagrees with the stricter school of P. Maas. But none of these scholars have formulated the principles by which long syllables may be equated with short in Pindar's text, or even those by which two short syllables may (...) take the place of one long and vice versa. Such attempts were made by great scholars of the past, notably by Erasmus Schmid, Hermann and Boeckh, but recent developments in metric and textual criticism have created a new attitude towards metrical problems and provided a new vocabulary. It is time that Pindar's practice was re-examined. Only from it can we deduce what his rules of composition were and find out what licences he allowed and what not. The question is important both for the textual critic and for the student of Greek metric. If we can discover the principles on which Pindar used such metrical licences, we shall be more qualified to consider emendations which involve them and to understand the rules which he set before himself when composing an ode. Oxidative damage to DNA has been associated with neurodegenerative diseases. Developmental exposure to lead has been shown to elevate the Alzheimer's disease related beta-amyloid peptide , which is known to generate reactive oxygen species in the aging brain. This study measures the lifetime cerebral 8-hydroxy-2'-deoxyguanosine levels and the activity of the DNA repair enzyme 8-oxoguanine DNA glycosylase in rats developmentally exposed to Pb. Oxo8dG was transiently modulated early in life , but was later elevated 20 months after exposure to Pb (...) had ceased, while Ogg1 activity was not altered. Furthermore, an age-dependent loss in the inverse correlation between Ogg1 activity and oxo8dG accumulation was observed. The effect of Pb on oxo8dG levels did not occur if animals were exposed to Pb in old age. These increases in DNA damage occurred in the absence of any Pb-induced changes in copper/zinc-superoxide dismutase , manganese-SOD , and reduced-form glutathion . These data suggest that oxidative damage and neurodegeneration in the aging brain could be impacted by the developmental disturbances. (shrink)
